QMP’s Guide to Utah EVS Certification

If a patient has reached out to you asking for a certification & you’re unclear about the process, take a look at this step-by-step QMP EVS certification guide.

Certifying a Patient

This certification process is for patients that have applied for their very first Medical Cannabis Card or who need to renew their Medical Card.

1.
Sign in at EVS.utah.gov
2.
Hover over “Tracking Inbox
a.
Click “QMP”
b.
Click “Your Name”
3.
Click on the “Certification/Recommendation” tab on the far right.
4.
Click “+ New” and be prepared to enter the following patient information:
a.
Date of Certification
b.
Last 4 of SSN
c.
First and last name
d.
Date of birth
e.
Qualifying condition
f.
Terminal illness (yes/no)
g.
Your QMP pin

Click “Save”
6.
Hover over the “Recommendation” tab
7.
Select “New Recommendation” and be prepared to complete the following:
a.
Select a Dosing Guidelines option
b.
Ensure Recommendation Date is populated
c.
Check the boxes next to the Dosage Forms you are allowing your patient
d.
Enter Medication & Treatment History as needed 
e.
Select "Yes" to set recommendation as Active
f.
Select "Yes" if you feel comfortable allowing the pharmacist to make changes to this dosage or select "No" if you'd like to approve all changes to dosage yourself.
8.
Click “Save”

Note: If you get an error message stating that there is no matching patient awaiting certification, reach out to your patient and verify exactly what is listed on their EVS application. Name, SSN, and birthdate have to match what is on the patient’s application in order to certify them. The patient must also be in Awaiting Certification status to complete this process or any other certification process.
