Step 1

Utah Medical Cannabis Card Application

To minimize wait time, we recommend starting your application with the state before your QMP visit. Complete the following steps to register for your Utah Medical Card.

1.
Create a login.utah.gov Utah ID account.
2.
Use that account to login to EVS.utah.gov and complete a new patient application.
a.
Hover over Tracking Inbox and select Patient.
b.
Fill out the requested information on the patient registration page.
c.
Once all fields are completed, scroll to the bottom of the page and hit the blue Save & Submit Registration button. This will change your registration status to Awaiting Certification.
3.
Let your clinic know that you’ve completed the new patient application, if you haven't already.

Once your QMP has certified your application on the EVS, you can proceed to Step 2. (At our clinic, this happens instantly!)

Step 2

Complete Your EVS Application

After your QMP enters your certification into the EVS system, you’ll need to complete a few steps before your Medical Cannabis Card can be processed with the Utah Department of Health.

1.
Login to EVS.utah.gov to pay your $15 application fee.
a.
Hover over Tracking Inbox and select Patient.
b.
Select the Payment tab.
c.
Hit +New and then the Click Here to Pay button.
d.
Complete your payment on the pop-up payment window that appears.
2.
You did it! Pat yourself on the back and keep an eye out for your Medical Card to be emailed from the state.

Pro-Tip: Make sure your pop-up blockers are off for this step!

Bonus Step

Renew Your Expired Medical Cannabis Card

So your Utah Medical Card expired, huh? Don't worry! Getting back into Active status is a breeze.

1.
Login to EVS.utah.gov to save and submit your application for certification.
a.
Hover over Tracking Inbox and select Patient (not New Patient).
b.
Click your name.
c.
Scroll to the bottom of your registration page and select the blue Save and Submit Registration button. This will change your registration status from Expired to Awaiting Certification.
2.
Let your clinic know you’re ready to be certified, if you haven't already. (Your QMP may require a clinic visit to set your card to Active.)

Once your QMP has certified your application on the EVS, you can follow the directions in step 2 again.
